We’re looking to hire a HEDIS Coordinator  for HEDIS measures. The main role of the HEDIS Coordinator  for HEDIS measures is to ensure all managed care patients are compliant with yearly preventive screenings. To provide excellent customer service to patients and facilitate outreach services and clinical services to our managed care population.

Job Description

  • Facilitates member outreach to ensure member receipt of important clinical services to improve member health outcomes.
  • Provides support for Healthcare Effectiveness Data and Information Set (HEDIS) quality initiatives and regulatory/contractual requirements including outreach to members who are noncompliant in the designated outreach services.
  • Supports effective medical record capture for HEDIS medical record review and abstraction of supplemental data.
  • Identifies potential resources and initiates collaboration with providers to improve member health outcomes and HEDIS measures.
  • Makes outbound calls to members to provide assistance in obtaining services (i.e., provide reminders, make appointments, arrange transportation, etc.) to avoid delays in treatment and improve member health outcomes.
  • Help identify when to refer member to Social Worker or Case Manager.
  • Develops and distributes educational information to members and providers in accordance with the HEDIS outreach work plan (mailings, social media, automated calls, etc.)
  • Assists in the development and standardization of outreach documentation and manages the data flow process to complete HEDIS documentation.
  • Documents member participation in outreach activities in accordance with abstraction and audit standards.
  • Conducts HEDIS provider chart request calls and mailings.
  • May develop and maintain member incentive programs to support HEDIS initiatives.
  • Records and documents all member and provider outreach activity.
  • Performs other duties as required.

Required Qualifications

  • Minimum 3 years’ recent and related health care marketing, outreach and/or health education experience.
  • Recent and related experience working with HEDIS criteria
  • Strong communication skills and experience speaking with medical providers.
  • Ability to work independently.
  • Ability to use standard corporate software packages and corporate applications.
  • Bilingual a Plus
  • Associate’s degree or equivalent experience

Detección de síntomas: al llegar a nuestras instalaciones, se proporcionará un cuestionario de salud antes de ingresar con preguntas sobre tos, fiebre y exposición al virus. Nuestro personal también es evaluado diariamente antes de llegar a nuestras instalaciones.

Restricting Visitors-We understand how difficult it could be to restrict our patients from bringing their companions/family members while they visit their physician in our facilities, but these important temporary measures will help minimize exposure, therefore, keeping the risk of infection low.

What to expect when arriving at our facilities.

All patients are screened at the entrance.

Masks are mandatory while in our facility, no mask-no entry.

Our Staff is required to wear all safety equipment, googles, mask and protective gowns to ensure yours and their safety.

Hand Sanitizer is available at the entrance and throughout the facility, we ask everyone to sanitize their hands when entering the office, the same way, our staff must sanitize their hands before and after a patient encounter.

Screening for Symptoms- When arriving at our facilities a health questionnaire will be provided before entering with questions about any cough, fever, exposure to the virus. Our staff is also screened daily prior to arriving to our facilities.

Temperature is taken at the door prior to entering the facilities.

Additional Safety Measures in our Facilities.

Common areas such as waiting rooms and restrooms are cleaned often, especially the frequently touched areas.

Exam Rooms are disinfected between each patient and extensively cleaned at the end of the day.

Waiting Rooms have been arranged to maintain social distancing, our furniture has been changed to create the space required between patients and we have placed markers on our furniture and floors to guide our patients where to stand and seat.

We want our patients to feel secure while in our facilities receiving the excellent and compassionate care they deserve while keeping them safe. EliteHealth follows all safety recommendations from the CDC (Centers from Disease Control). Our patient safety is our greatest concern and all we do is with you in mind.
